What is a "preferred provider"?
CuraNet has created a Preferred Provider Organization (PPO) Network and the providers in our network are your "preferred providers" for medical care. A preferred provider can be a pre-approved hospital or other healthcare provider who has agreed to offer their services at an established price.


How do I find a network provider?
By using the Provider Search you are able to search for a provider based on location, name, facility or specialty type. Additionally, the status of your doctor in our network can be confirmed by contacting CuraNet's Customer Service Referral link at our toll free number 1-888-354-5470. Customer service representatives are available Monday through Friday from 8:00 a.m. to 5:00 p.m., Eastern Standard Time.


Can I go to a non-network provider?
Yes, you may visit an out of network provider, however your benefits may be reduced as a result which will mean more out-of-pocket expense for you. Check with your Human Resources Department or review your health plan.


I'm a patient/member. How do I get an ID card?
Please contact your claims administrator or your employer’s Human Resource office directly.

Do I need to carry my ID card with me at all times?
Yes. In order to receive the CuraNet preferred discount rates you must present your ID medical card at the time of service. Providers may not honor the discount if your ID is not presented at time of service, resulting in more out-of-pocket expenses for you.


I am a policy holder. How do I submit a bill or claim for payment?
In most cases, the physician or hospital will file the bill in your behalf. However, if you need to submit the bill yourself, the address of where to submit your bill is located on your CuraNet medical ID card. For faster services, you may call the number printed on your medical ID card and ask for the fax number of where to fax the claim. All claims should be submitted to your insurance company. CuraNet does not pay claims or have access to your benefit plan information. To learn more, click here.


I am unable to locate my provider or hospital in the Provider Search. What should I do?
Call CuraNet at 1-888-354-5470 to verify provider participation. You may nominate your provider by submitting a Provider Nomination form.
