Q: How often is provider information updated?
A: The provider information that we display is updated nightly. It is provided by either Cactus (Credentialing Database) or Diamond (Contracting and Claims Database), which are HAP’s two main database systems. The information provided is updated by 2 different venues as follows:
- HAP’s Credentialing Department updates and/or verifies provider information every two years.
- If a provider, member or guest notices that information is outdated or incorrect, they can contact CuraNet by phone at (888) 354-5470 or by email at curanetweb@curanet.org and then we will work with the appropriate departments to correct the error.
Q: How do I create a Provider Directory online?
A: Follow these simple steps:
- You can search for a provider by starting on the Provider Search page.
- It is required to select the type of provider that is needed (personal care physician, specialist, etc).
- You have the option to enter the provider name that you would prefer.
- Either click "Find Providers Now" to see your results, or click "Additional Search Criteria".
- Clicking "Additional Search Criteria" will allow you to enter additional search criteria such as: the preferred distance you are willing to travel to this provider, any preferred office hours, and the gender, language and hospital admitting privileges that you would prefer this provider to have.
- Click "Find Providers Now".
- Your results will appear. You may view the search results online or have your results sent to you by e-mail through the Download to PC feature.
Q: What types of providers are listed under “Personal Care Phys”?
A: "Personal Care Phys" displays the following types of providers:
- Family Practice
- General Practice
- Internal Medicine
- Pediatrics
Q: What types of providers are listed under “Other”?
A: “Other” displays ancillary providers. These are additional providers who supply other needed services such as:
- Durable Medical Equipment
- Home Health Center
- Hospice
- Laboratory
- Skilled Nursing Facility
Q: How do I search for a physical therapist or a physical therapy facility?
A: Starting on the Provider Search page, select "Specialist" as the type of provider and hit "Additional Search Criteria." Then select in Specialty Type using your mouse or the CTRL key any of the following specialties related to physical therapy that best fit what you are searching for:
- Physical Medicine and Rehabilitation
- Neuromusculoskeletal Medicine
- Occupational Medicine
- Osteopathic Manipulative Therapy
- Pain Management
- Pediatric Rehabilitation Medicine
- Rehabilitative Medicine
- Sports Medicine
Q: How do I search for an Obstetrician or a Gynecologist?
A: Starting on the Provider Search page, select "Specialist" as the type of provider and hit "Additional Search Criteria." Then select from specialties including "Gynecology", "Obstetrics" and "Obstetrics & Gynecology" in the Specialty Type list using the CTRL key or your mouse.
Q: Why do Year of Graduation, Internship, Residency and Fellowship appear blank on some of the Provider Details pages?
A: This information is unavailable to display for CuraNet providers at this time. However, if the provider is also a HAP provider who has given consent through HAP.org, this information will display.

Purpose
The Provider Search application allows users to search for providers or facilities using several search parameters. From the Search Results, you can:
- View provider or facility information
- Compare providers or facilities
- Create a directory in an Adobe acrobat PDF format for all providers or facilities which fit your search criteria.

Page Descriptions
Additional Search Criteria
The Additional Search Criteria page allows you to narrow your search by distance, specialty type or other optional search parameters.
You may enter or select information into the following fields:
- Search by Distance
- ZIP code - a code of letters and digits added to a postal address to aid in the sorting of mail
- Specialty Criteria:
- Specialty Type - the name of the provider's specialty in order to narrow your search
- Additional Criteria:
- Gender – one’s sexual identity, female or male
- Other Languages Spoken – additional languages the provider is fluent in, not including English
- Hospital Admitting privileges - presents a list of hospitals to which a provider may admit patients
- Board Status - Board Eligible or Board Certified
- Evening/Weekend Hours – displays whether the provider has evening and/or weekend hours
Note: These criteria have a default value of "No Preference."

- Find Providers Now! – navigates you to the Your Search Results page where you can view a list of Providers that fits your criteria

Provider Comparison
The Provider Comparison page gives you the ability to compare up to three providers or facilities at one time. To use the Provider Comparison feature, conduct a provider or facility search from the Provider Search page. Then, select up to three check boxes for each provider or facility you wish to compare and click the Compare button.
Note: For information that is difficult to keep up-to-date, we advise you to verify the information with the provider for certainty. Examples of information that you may want to do this for include office hours and accepting new patients.
The following data may be compared on this page:
- Provider Name and ID – the provider’s name and unique provider identifier number
- Specialty – the branch of medicine a provider specializes in
- Address & Phone – the address(es) and telephone number(s) of the provider’s office(s)
- Office Hours – the time range when the provider’s office is open for patients
- Gender - one’s sexual identity, female or male
- Additional Languages - additional languages the provider is fluent in, not including English
- Education - The school where the provider attended classes to learn about the field of medicine before starting to practice medicine
- Board Status - Board Eligible or Board Certified
- Show Hospitals - this link will take you to the Hospital Screen, which lists those hospitals where the provider may admit patients. Currently, not all CuraNet providers are linked to a hospital at which they have privileges.
Note: The Provider Comparison page will display the office address and office hours selected from the Your Search Results page. To see all office addresses and office hours associated with a provider, select the provider's name.
- Accepting New Patients - Provider accepts both new patients and current patients who change from other health plans to CuraNet
- Medical Group Affiliation - A medical group with which the provider has a relationship or association.
You have access to the following functions on this page:
- Download to PC - download your search results to an Adobe acrobat PDF file sent via email
- You will need to have Acrobat Reader installed on your system to view the PDF.
- New Search - returns you to the Provider Search page where you may begin a new search
- Back to Results - returns you to Your Search Results page
- Michigan Department of Community Health - opens a new window to the Michigan Department of Community Health website where you may search for sanctions on record with the state of Michigan
- Provider Name - opens a new window with detailed information about the selected provider or facility

Provider Details
The Provider Details page allows you to read information about a provider or facility. The functions on the Provider Details page are determined by the type of provider.

| Professional Degree |
An academic degree that is examined and awarded by prescribed schools and universities, and intended for those wishing to follow a given profession. Some examples of professional degrees are:
- MD: Medical Doctor - The degree a person receives after completing four years of medical school. A medical doctor is licensed in the science and art of dealing with the maintenance of health and the prevention, alleviation, or cure of disease. After completion of medical school, a medical doctor chooses an area of medicine to specialize in and completes a residency.
- OD: Doctor of Optometry - A specialist licensed to practice the art or profession of examining the eye for defects and faults of refraction and prescribing especially corrective lenses or exercises.
- DO: Doctor of Osteopathic Medicine - The degree a person receives after completing four years of medical school. A doctor of osteopathic medicine is licensed in the system of medical practice based on a theory that diseases are due chiefly to loss of structural integrity, which can be restored by manipulation of the parts supplemented by therapeutic measures (as use of medicine or surgery). After completion of medical school, a doctor of osteopathic medicine chooses an area of medicine to specialize in a residency.
- DPM: Doctor of Podiatric Medicine - A specialist licensed in the medical care and treatment of the human foot.

- Qualified to take a test by a medical specialty board

- Show Specialists - lists the specialists to which the provider may refer you. Please note that CuraNet is a PPO and there are no network limitations on where members may go for care.
- Get Google Map - opens the Google Maps page in a new window. The provider or facility’s address is pre-populated. You may add/modify your address if you wish.
If the provider is a Specialist, you have access to the following functions on this page:
- Show Hospitals - lists the hospitals to which the provider may admit you within your network. Currently, not all CuraNet providers are linked to a hospital at which they have privileges.
- Show PCPs - lists PCPs that can provide a referral to the given specialist. Please note that CuraNet is a PPO and there are no network limitations on where members may go for care.
- Get Google Map - opens the Google Maps page in a new window. The provider or facility’s address is pre-populated. You may add/modify your address if you wish.

Provider Search
The Provider Search page is accessible for all users. This page allows you to select search parameters.
You may enter or select information into the following fields:
- Provider Last Name or Facility Name - Provider last name or facility name for search. If inputted, must include a minimum of the first 2 letters.
- First Name - First name of the provider. This is not applicable to facility names.
- Type of provider - Provider types are PCP's, Specialists, Vision, Pharmacy, Hospitals, Other (this type of provider includes ancillary providers such as labs), and Urgent Care.
You have access to the following functions on this page:
- Additional Search Criteria – navigates you to the Additional Search Criteria page where you are able to narrow your search by distance, specialty type or other optional search parameters.
- Find Providers Now! - navigates you to the Your Search Results page where you can see a list of Providers that fits your criteria
